Research details
Areas of research interest
SS-31 is studied for diseases involving mitochondrial dysfunction, including inherited mitochondrial myopathies and Barth syndrome, and more broadly in aging research. It is the first mitochondria-targeted compound to reach FDA approval. Elamipretide is the active ingredient in an FDA-approved prescription medicine. This research-grade material is not that product and is not for human use.
Overview
SS-31 (elamipretide, D-Arg-Dmt-Lys-Phe-NH₂) is a synthetic tetrapeptide that binds cardiolipin, a phospholipid in the inner mitochondrial membrane. Binding cardiolipin is proposed to stabilize mitochondrial membranes and support energy production.
Research summary
Barth syndrome. Barth syndrome is a rare inherited disorder of cardiolipin metabolism. In the randomized TAZPOWER trial, elamipretide improved knee extensor muscle strength, which formed the basis for FDA approval [1]. FDA requires a confirmatory trial showing that this strength gain translates into patient benefit [1].
Mitochondrial myopathy. In the Phase 3 MMPOWER-3 trial of 218 patients with primary mitochondrial myopathy, elamipretide did not meet its primary endpoints [2].
Regulatory status. FDA granted accelerated approval in September 2025 for improving muscle strength in Barth syndrome patients weighing at least 30 kg [1].
